Consent for the Processing of Personal Data by the National Health Fund for Conference Participants within the “EU4H Project Grants: European Joint Action Mental Health Together – MENTOR”

(for participation in the MENTOR Stakeholder Board Meeting organised within the European Joint Action “Mental Health Together” – MENTOR)

I, the undersigned, declare that:

I have read and understood the information clause regarding the processing of personal data by the National Health Fund (NFZ) for the purposes of organising and conducting the MENTOR Stakeholder Board Meeting on 30 January 2026.

I voluntarily consent to the free-of-charge and time-limited processing of my personal data by the National Health Fund, headquartered in Warsaw, for the duration of the Project and its sustainability, as defined under Article 4 (1) GDPR, in the following scope:

1. Personal data: first name, surname, academic title (if applicable), job title, organisation/affiliation.
2. Contact details: e-mail address (solely for communication related to participation in the Meeting).
3. Image and voice: data arising during the online meeting. The meeting will be recorded exclusively for the purpose of preparing the minutes.

I consent to the use and storage of the above personal data for informational and promotional purposes related to the MENTOR Project, in accordance with the information clause.

I acknowledge that my personal data may be made available through the Project’s communication channels, including online media.

I understand that I have the right to withdraw my consent at any time, without affecting the lawfulness of processing based on consent before its withdrawal.
