Definition of the Integrated Care Pathway (D5.4.2)
The report defines a European integrated, person-centred mental health and social care pathway focused on social inclusion and quality of life. It describes core components such as multidisciplinary teams, multidimensional assessment, co-designed personal health budgets, cross-sector interventions and a shared monitoring framework for implementation.

The mapping review analyses how European countries implement a Mental Health in All Policies (MHiAP) approach, drawing on a literature review, a survey and interviews with 14 Member States. It identifies key enabling factors, governance arrangements and capacity-building needs, and informs the development of a practical guidance and self-assessment tool to support cross-sector integration of mental health in public policies.

Valorising current good practices of PLE involvement (D5.3.1)
The report analyses current practices of involving people with lived experience (PLE) in mental health policies and services across participating countries. It identifies effective models, enabling conditions and barriers to meaningful participation and supports future training, pilot actions and policy recommendations for strengthening co-production.

Guide to submitting mental health practices to the MENTOR repository (T5.2.2)
The manual explains how community-based and digital mental health practices can be submitted to the MENTOR repository. It presents eligibility and evaluation criteria aligned with the European Best Practice Portal, the three recognition levels (promising, good and best practices) and the full submission and assessment workflow.

This standardised submission form structures the collection of information on mental health practices, including background, objectives, methods, outcomes, equity, sustainability and conflicts of interest. It supports comparable evaluation of practices and their classification as promising, good or best practice within the MENTOR repository.

Knowledge exchange report (D5.4.1)
The report summarises the results of a multi-country knowledge-exchange process based on a structured survey and expert panels. It analyses needs, services and policies related to integrated mental health and social care pathways, including personal health budgets, and supports mutual learning and evidence-based policy development across participating Member States.

The guidelines define a person-centred Health Budget model for mental health services, describing qualifying and operational elements, multidisciplinary assessment, governance and administrative pathways, and the integration of health and social care to support personalised life projects, social inclusion and recovery-oriented community-based care.

MENTOR – Mental Health Together
The MENTOR Joint Action supports the promotion of mental health and well-being across Europe through cross-sector policy development, innovative digital solutions and the inclusion of people with lived experience. The project develops guidance and tools for implementing a Mental Health in All Policies approach, strengthens integrated care pathways, supports youth prevention and promotes collaboration and synergies with other EU initiatives.
